ImPACT Passport® empowers you to take an active role in concussion management. Provide healthcare professionals access to your data, record symptoms, and locate an ImPACT-trained care provider anytime, anywhere. Grant care providers immediate access to your test results and symptom records by providing them your Passport ID, a unique identification code received upon completion of an ImPACT®, ImPACT Pediatric®, ImPACT Quick Test®, or Cognitive Impairment Screener™ (CIS).
ImPACT Passport includes the ability to:
• Record ImPACT Passport IDs
• Locate ImPACT-trained concussion care providers
• Record and upload symptoms to your profile

This app is horrible. We put in all of the info and it said there was "an error connecting to impact's servers; please check your internet connection" but our connection is fine because every wireless device in our home is connected. Also, the description is misleading because it says the app works on all devices but only gives the option for a smartphone or tablet and not laptop or desktop. All devices should mean all, not some. If I could have rated this zero stars, I would have.

I really appreciate knowing that my family has a plan to prevent my son from returning to play too soon, if he were to have a concussion. The app keeps his Passport ID safe and available on my phone. Installation was easy, and it helps me feel prepared for the unexpected.

This app make more complicated, I put the Passport ID many times and this app keep telling many many times " the passport ID not is correct
